Do I qualify for a wheelchair assessment?

You may be eligible if:

• You are registered with a Birmingham GP
• You need a wheelchair for more than 6 months
• You intend to use the wheelchair five or more times per week, for at least an hour at a time
• You have a medical condition that affects your ability to walk indoors or outdoors

If you meet all these criteria, you should complete a wheelchair service referral form.
