Birmingham Wheelchair Service
Do I qualify for a wheelchair assessment?
You may be eligible if:
- You are registered with a Birmingham GP
- You need a wheelchair for more than 6 months
- You intend to use the wheelchair five or more times per week, for at least an hour at a time
- You have a medical condition that affects your ability to walk indoors or outdoors
If you meet all these criteria, you should complete a wheelchair service referral form.
How to refer
You can request a paper referral form by contacting the service or download it from their website.
