How do I request a Fit Note (Sick Note) or extension?
If you have been ill for 7 days or less, you do not require a doctor's Fit Note and can self-certify. If you have been off work for more than 7 days, you can request a new or extended Fit Note using our online admin request form on our website.
What specialized clinics and long-term condition reviews do you offer?
We offer comprehensive nurse-led management for long-term conditions including asthma, COPD, diabetes, and hypertension, as well as routine cervical screening, routine childhood immunizations, and family planning services.
How do I request a private medical report, insurance form, or GP letter?
Non-NHS services (such as medical reports, insurance claim forms, and private letters) are not covered under NHS General Medical Services and incur a standard fee. Please submit your request via our online admin form or at reception, allowing up to 10–14 working days for completion.
Do you provide travel vaccinations?
We provide routine NHS travel vaccinations (such as Hepatitis A, Typhoid, and Tetanus/Polio/Diphtheria). Please submit an online travel assessment form at least 6 weeks prior to travel so our nursing team can review your itinerary and vaccination history.
